Please include the name and year of the student, any symptoms he or she is experiencing as well as the date of when symptoms began, the date of his or her COVID test, any school sports or activities he or she attended and when he or she last attended, exposure information such as how you think exposure occurred (social, family, household or unknown). Please include a copy or scan of the test results as well.
