Sports Physicals
Physicals must be completed and on file in the Nurse's Office by August 1. Failure to have your physical on file will delay your child’s opportunity to participate in tryouts.
Physicals are valid for 13 months. It is therefore wise to have your physical completed in June or July so that physical covers the entire school year. To be an athlete at Loyola, students must have to have a physical every year that they play a sport.
North Shore University Health System will be offering Sports Physicals to sophomore, junior and senior athletes at the Glenview Park Center this summer through August 16. A discounted rate of $50 will be offered if you elect to not process through your health insurance. Call 847.657.3540 to schedule and appointment with Dr. Jaworski and be sure to mention "Loyola Sports Physical" when calling.
